Merit-Based Scholarships (1 recipient)
- Award Amount: Between $2,500 and $5,000 distributed over four years.
- Focus: This scholarship reward high-achieving student who demonstrate academic excellence along with outstanding community service and leadership in areas related to the foundation’s values.
- Eligibility Criteria:
- Applicants must demonstrate excellence in academics, extracurricular involvement, and a commitment to community service, particularly in heart health or other areas connected to the foundation’s mission.
- Applicants must submit a 500-word essay showcasing their community service impact in heart health or explaining their connection to the foundation’s mission. Proof of academic transcript is required. GPA 3.75 or greater. Attending a 4-year college.
- Award Amount: Ranging from $1,500 to $4,000 distributed over four years.
- Focus: This scholarship is designed to help student(s) who might otherwise be unable to afford higher education by offsetting tuition costs, books, and living expenses.
- Eligibility Criteria:
- Applicants are required to provide financial need documentation (such as FAFSA results or income verification).
- Additionally, applicants must submit a 500-word essay explaining how receiving the scholarship will support their educational goals. Proof of academic transcript is required. GPA 3.0. Attending a 4-year college.
- Award Amount: Between $2,000 and $4,000 distributed over four years.
- Focus: This scholarship is tailored to student(s) entering specific fields (for example, nursing, allied health, mental health, social work, or community health) where the foundation’s mission is particularly relevant.
- Eligibility Criteria:
- Applicants must provide proof of enrollment in an accredited program in their chosen field.
- They must also demonstrate a passion for making a community impact and submit a 500-word essay outlining their career goals.
- Applicants should provide a 500-word essay or personal statement detailing their community projects, leadership roles, and future for community engagement. Proof of academic transcript is required. GPA 3.5 or greater.
- Attending a 4-year college
- Award Amount: Range from $500 to $1,500 (One-time award).
- Focus: This one-time award is designed for students seeking to offset costs related to enrollment in a community college or trade school program.
- Eligibility Criteria:
- Candidates must submit a 500-word essay explaining how this award will help jump-start their career goals. Proof of academic transcript is required. GPA 2.75 or greater.
- Attending a community college or Trade school
